Disappearing Mail in Outlook

I have a user who has a problem with some email disappearing as soon as they
hit the inbox. This seems to only happen when the mail comes from certain
internal users, me being one of them. I can send an email from my Outlook, go
to her Outlook, hit send/receive, watch the email hit the inbox and
immediately disappear. A few seconds later I get the new mail notification
bubble. I click on it and I get an error that says "operation failed".

I opened OWA and was able to see the email I sent even thought it didn't
show up under Outlook. I also opened the Admin Outlook on the server and the
email was not there either.

We use cached mode for all clients except the admin Outlook.

I have checked all the rule and views but these do not seem to be the issue.

I have scanned the .ost file for errors but none are reported. I have
renamed the .ost and let Outlook create a new one but still have same issue.

This issue happened on her old PC and is now happening on a freshly
formatted PC.

Any suggestions?

did you try to launch outlook with /cleanrule to remove all rules ?
If the mail appear and then go, should be a zombie rule

try to see if you have set the filter on show only new messages.
