Files delete on left mouse click

I have Windows 8.1 on an HP Notebook. My system was working fine for the past 1 year till one day while left clicking on a folder in a portable drive I got a confirmation whether I wanted to delete. I had to cancel around 10 times. Since then I face the intermittent issue that clicking on a file will delete it (It will go to recycle bin). Clicking on an empty folder will delete it and if the folder has files will ask me for confirmation many times whether I want to delete it. After a gap of few minutes the issue will not be there for the next few minutes until it comes back again. When the issue comes even what I have written on sticky notes will get deleted without me clicking on anything. Restarting the system may sometimes help but only for a few minutes.

The issue gets resolved for a few minutes whenever I run the following command (which I found somewhere online): regsvr32 /i shell32

I checked with 2 different Anti Virus and also Malwarebytes if there was any malware. Could not detect a thing. Still suspecting a malware, I deleted all my drives (merging them and formatting keeping only C and D drive) and then reinstalled Windows 8.Still when I logged in on first boot, on clicking a newly created folder it got deleted.

Can this be a malware issue considering I have formatted all the drives and don't have anything except Windows. Or is it a hardware issue like issue with delete button or mouse (which somehow gets resolved temporarily when i run the command: regsvr32 /i shell32)?
