Is it possible to pop up input box for two values at the same time?

For example, I would like to ask user to provide FirstName and LastName, how should I do that? Should I create a separate input form?

Or is there a simple way (such as input box)? But I don't want to ask one input value at a time, since it may annoy user. If not possible, then probably I should go with a separate form (asking for FirstName and LastName).
