W2K3 R2, IE6SP2 - article 840378 describes the issue perfectly, except for
the file dates & versions are newer than those listed for the hotfix.

Basically, using roaming mandatory profiles with GPO to set the proxy
settings and homepage URL - user logs off and three folders remian behind
contaiing an index.dat file. Every time the user logs on a new profile folder
is created - username.domainname.XXX.

thanks.

Have you tried installing UPHClean?

Yep, UPHC 1.6.30 is installed.

Does anyone know if this behavior also occurs with IE7? I haven't tried IE7
yet as I've read that it doesn't respond so well to the IE GPO settings for
proxy settings, etc.

You can set the GOP to delete the local profile on logoff. Have you
tried that?
