Enter Network Password Window pops up

Everyday the Enter Network Password Window pops up indicating my server, user
name, and password (in stars). This started about a week ago and I don't know
why or how to fix it.

I'm using Office Basic Edition 2003 & Windows XP
Please advise

Check your host file - it can be read in notepad.
